Questionnaire responses
Your selected answers and current position are stored in your own browser so you can resume the 102-item questionnaire, including the 12 developmental magnitude items. The questionnaire does not submit those answer values, domain scores, magnitude index, or finished profile to an OMAT assessment database.
Control and deletion
Choose “Start over” or “Retake assessment” to remove saved questionnaire progress from that browser. Clearing browser site data also removes it. Progress does not automatically move to another browser or device.
Optional name and email
The name and email fields are entirely optional. If entered, those values remain only in the current open page and label the finished result and copied summary. They are not written to saved questionnaire progress, submitted to OMAT, or restored after the page is refreshed or reopened. A printout, saved PDF, or copied summary will contain them if you choose to include them.
Profile Passport and ensemble planner
When you choose “Place this profile in the ensemble planner,” the page places the three T/P/E percentages, profile title, listening-mode label, magnitude, and optional name in temporary session storage and reads it on the planner page. Email and item-by-item answers are never included. The temporary copy is removed after import. A downloaded Profile Passport is a JSON file saved by your browser and remains under your control. The planner itself keeps member names, scores, and project details only in the open page.
Audio and accessible fallback
The listening tasks request ordinary audio files from the website. They do not use a microphone, camera, contact list, or music library. You may mark any listening task “Unable to assess.” That task is omitted, and the result is labeled partial-listening or self-report-only so it is not mistaken for an equivalent complete profile.
Anonymous aggregate counters
OMAT counts a limited set of actions: page views, questionnaire starts and completions, result copies and prints, Profile Passport transfers and downloads, planner imports, group-brief copies and prints, and paper-link openings. The event endpoint accepts only those fixed labels. It stores one total per event per UTC date—no raw answers, T/P/E values, magnitude, name, email, project, member entry, page address, browser identifier, user identifier, or cookie. These counters cannot reconstruct an individual session or profile.
Hosting requests
The site is delivered through OpenAI Sites infrastructure built on Cloudflare services. Standard HTTP requests necessarily expose network and device information such as IP address and user-agent to the delivery infrastructure, which may process operational and security logs under its own service controls. OMAT’s aggregate event table does not copy that request metadata. This notice does not claim a provider retention period that OMAT has not independently published.
Printing and saving
“Print / save result” and field-guide print buttons use your browser’s print tools. OMAT does not receive a copy of the PDF or printout you create unless you later choose to send it.
Email and future research
If you email OMAT, your message is handled through ordinary email systems. Completing this public questionnaire is not enrollment in a research study. Any future study would require separate information, eligibility criteria, consent, governance, and data-handling terms.
